We provide structured care for all patient's with long term illness. This care is tailored to meet the needs of the individual patient and their condition.
Asthma
Asthma is a condition that affects the airways, the small tubes that carry air in and out of the lungs. The airways become oversensitive causing the muscles around the wall of the airway to tighten up, making it narrow and difficult for air to flow in and out. The lining of the airway then swells and a sticky mucus is produced.
It is a common and potentially serious chronic disease that imposes a substantial burden on patients, families and the community.
Ireland has the 4th highest prevalence of Asthma worldwide.
There has been an overall upward trend in the prevalence of Asthma.
As a practice we will aim to follow the recommendations of the Global Initiative for Asthma. We would hope to reduce the frequency of acute exacerbations of asthma through effective education on management and therefore improve levels of control of asthma in our patients.
We are registered to provide Free GP care to the Under 8’s. As part of this contract we will provide the cycle of care for asthma management.

Diabetes Mellitus
Diabetes is a common chronic, progressive health condition. It is a condition where the amount of glucose in the body is too high. This is because enough insulin is not made or the insulin cannot be used very well.
The International Diabetes Federation Diabetes Atlas estimated that there were 207,490 people with Diabetes in Ireland in the 20-79 age group, a prevalence of 6.5% in the population.
In September 2015 a GP based diabetes management programme was introduced. The aim of this programme is to reduce the mortality and morbidity from diabetes.
As a practice we believe in the need to promote prevention of diabetes, early diagnosis and the need to have a structured approach to the management of diabetes.
